The Primary Care Network (PCN) aims to align primary care with the NHS Long-Term Plan, which focuses on increasing healthcare staff in GP practices to ensure patients can see the right professional for their needs.
GP practices were encouraged to collaborate with others in their area, creating a shared patient population of 30,000 to 50,000. This collaboration led to the joint efforts between Church Street and Newbury Street Practices.
The partners, practice managers from both practices and the newly appointed operations manager form the Executive Committee for Wantage PCN. They meet monthly to review and agree on the PCN’s strategic development, finances, and key performance indicators (KPIs), and to discuss significant contract updates.
The Wantage PCN team currently includes 2 clinical pharmacists, 1 pharmacy technician, 1 physiotherapist, 2 social prescribers, 1 paramedic, 1 mental health practitioner, and 1 health and well-being coach.
