Accessing Your GP Health Record

Starting November 2023, you will automatically have access to more detailed parts of your GP health record, including appointment notes, letters, and test results.

If you wish, you can request full access before this date by contacting the practice directly.

How to Access Your Record

For Patients

Register and verify your identity using the NHS app or website.

Once completed, you’ll be able to view your health record online.

If you do not have a photo ID to verify your NHS identity, you may contact us using the button below to get you verified using GP Linkage Key. See how to prove who you are without a photo ID

For Proxies

If you’re accessing records on behalf of someone else (such as a child or an adult who has given consent), you must have legal authority or documented consent.

Parents can usually access records for children under 11, but restrictions apply for older children to protect their privacy.

From age 16, patients typically manage their own access unless specified otherwise.

You can get proxy access even if you are not our patient

Please note

Children’s Records: If you are a parent, you can generally access your child’s records until they turn 11. For children aged 11 and over, access may be restricted to protect their confidentiality. From 16, the individual typically manages their own access.

Early Access: If you prefer to gain full access before November 2023, you can request this by reaching out to the practice.
